Studies have demonstrated over 15% absolute risk reduction in death and neurologic injury using this therapy. Although hospitals and medical centers have become familiar with this important intervention it still remains greatly under utilized due to an experience and lack of resources to safely and effectively deploy such a program. The objective of this book is to educate and familiarize both providers and institutions as to how to develop and deploy and provide therapeutic hypothermia to their patients. The current knowledge for this is provided by speakers and national experts and also by literature review. There are several courses being provided on this as well throughout the US. These are good venues for people to come and see and get hands on experience, but there still needs to be a concrete book with references on how to go about getting this program started.

Book Description: Problem: Effective therapeutic hypothermic treatment involves quick and careful management in order to optimize neurological outcomes and improve the morbidity and mortality associated with cardiac arrest. Currently there is no standardized guideline in implementation of therapeutic hypothermia across institutions throughout the country. There are many studies that look into this treatment; however, the sample sizes have been small and many confounding variables have affected outcome. This literature review aims to look at recent studies involving different aspects of treatment such as target temperature of cooling, method of cooling, and investing new novel techniques in cooling treatment that are used to improve patient outcomes. Methods: The search for articles was performed using the Weill Cornell Medical Library to access PubMed using the following search terms: "therapeutic hypothermia" and "brain", "cardiac arrest" and "brain", "target temperature" and "therapeutic hypothermia", "methods of cooling in therapeutic hypothermia." Results: The search for "therapeutic hypothermia" resulted in 23,492 results while "therapeutic hypothermia cardiac arrest" yielded 4903. Many of the results were in - depth discussions about side effects, techniques of measuring cooling, and pediatric cases which were not as relevant to this review. A large amount of literature has been recently published as hypothermic treatment has been studied for many years; however, continued detailed research helps move its use into the spotlight. Overall 17 articles were used which detailed clinical trials in which hypothermia has been used, its outcomes, and effect on the brain. Conclusions: Though there have been many studies that have looked at the implementation of therapeutic hypothermia in cardiac arrest, there needs to be further evidence -based medicine in order to standardize treatment guidelines. Larger sample sizes should also be taken into account in order to capture a larger patient population that can benefit from these treatments. The goal of the following proposal will be to look into an area that has not been studied as extensively.

Book Description: Background and Aims:Therapeutic hypothermia is one of the most perspective neuroprotection strategies. Therapeutic hypothermia with the use of cooling helmet device is superficial artificial hypothermia technique.We analyzed the effectiveness of the therapeutic hypothermia in ischemic stroke for highlighting perspective groups of patients. Methods:Total amount of patients was 107 acute ischemic stroke patients (hypothermia group -87 patients; control group - 20 patients). Inclusion criteria was acute ischemic stroke first 72 hours onset; NIHSS >4 points. Exclusion criteria was bradycardia, conducted trombolysis or/and thrombectomy. We used the cooling helmet device for hypothermia induction. The effectiveness of the therapeutic hypothermia was evaluated on 14 day of the stroke onset (mRS points). Results:The effectiveness of the therapeutic hypothermia depends from time between stroke onset and the hypothermia induction (r=0.43). The therapeutic hypothermia was successful when it had been started before 48 h from stroke onset. mRS score in hypothermia group was 2.58u00b11.92; in control group u00ac u20144.25u00b11.61 points (p=0.0005). The mortality rate in the hypothermia group was 9,1 %, and in control group it was 25%, (p=0.0256).The therapeutic hypothermia was effective both in atherothrombotic mRS 1,83u00b11,58 points) and cardioembolic (mRS3,34u00b11,96 points) stroke subtypes , but it was more effective in the atherotrombotic subtype patients(p=0,0002). Hypothermia was more effective in anterior circulation stroke.Conclusions:The effectiveness of the therapeutic hypothermia is reduced over time. The therapeutic window for this method is first 48 h from stroke onset. The therapeutic hypothermia reduce mortality and improve functional outcome in stroke patients on 14 day from stroke onset.
